Yes, supplements like Elk Velvet Antler that are derived from an animal source will have protein in them. However, when used as a supplement, at the relatively small serving size that you will use, this will not impact the amount of protein that you feed your dog. As far as feeding low protein for a dog with kidney disease, this is largely an outdated approach as modern research does not recommend limiting protein until the final stage of kidney failure. The reason for this is that reducing protein will negatively impact skeletal muscle and the heart (which is a muscle). Lastly, Cranberry can be helpful at times, however for a dog with CKD I would be cautious with its use due to its content of oxalates that can be irritating to the kidneys.
