The myceliated brown rice that you address in your question is the growing medium for the mushrooms so is not a filler. In higher-quality mushroom supplements like ours the growing medium has been broken down by the enzymes produced by the mycelium as the mushrooms grow and is rich in nutrients as a result. Any carbohydrates originally in the brown rice will have been broken down, or "myceliated", and used as food for the mushrooms so will not cause a blood sugar spike and will be fine for your diabetic cat.
